Memorial Bench Requests

 Town of Carman Memorial Bench Options are:

 Option 1 Bench 

Option #1 – Prairie Grass Bench. 4’ Steel Bench with Prairie Grass Design. $2,000

  • Environmentally friendly
  • Attractive and easily maintained because they resist fading and do not need staining
  • Made of high-density polyethylene consisting of 90% post-consumer waste
  • The bench donation costs covers the material, and installation.

Bench_Option_2

Option #2 – Pathway Bench – 4’ Composite Bench - $1,000. (Based on Availability -If Supplier runs out or changes the bench may be altered as it is not a local vendor.)

  • Constructed of Polywood lumber that is not prone to splinter, crack, chip, pel or rot.
  • Resistant to stains, corrosive substances, and other environmental stresses

Benches can be placed in community parks and green spaces and along pathways. Donors will work with municipal staff to identify an appropriate location.  **The location is a preferred location, but the Town may move the bench if it is required for safety, or maintenance purposes.

 Commemorative Plaque

Bench dedication plaques measure 3” x 5” and include up to 4 lines of text; each line comprising up to 16 characters.  Characters include letters, spaces, and punctuation.  Larger or capital letters may take up more space.  We use one standard font but can also incorporate italics.  Text size of shorter inscriptions can be increased to use the entire available space.  No images or logos will be engraved

 How to Request

A separate application form is required for each bench, even if the request is being submitted by the same requestor to submit a request for a commemorative bench:

Staff will provide an initial response by email within two weeks of submission.  This response will confirm that the application was successfully received and has been placed in the queue for review.

Application Review

Staff will review applications and will respond to applicants within 3 weeks of application receipt. The application review includes site-specific assessments by staff.

In their response staff will:

  • Confirm installation location or suggest alternative locations
  • Verify the requested wording on the commemorative plaque

Payment Request

After the location for installation is confirmed, applicants will receive a payment request by email. Payment in full is required before approval of the bench order and plaque order will begin.

Installation

  • Whenever possible, benches will be installed within the calendar year of the request.
  • Applicants will be contacted if requests cannot be completed within the calendar year.
  • Bench Installation dates cannot be specified due to manufacturing, weather, tree supply, and park construction.
  • Donors will receive a written confirmation, location description and a photo once the bench plaque installation has been completed.

 Certificate

If requested on the application form, donors will be mailed a certificate to recognize their commemorative donation.

 Tax-deductible Receipts

The donation is tax-deductible through the Income Tax Act , Sub-paragraph 110(1) (a)(1v), and on request only.

  • Requests for tax-deductible receipts are accepted until December 1 each year.
  • Email for the Donors’ Declaration form.

Warranty

The municipality will replace or repair the items, at no charge to the applicant for the warranty period specified:

  • Plaques within two (2) years of the original installation date
  • Metal Commemorative benches with ten (10) years of the original installation date
  • PolyWood Commemorative Benches with (5) years of the original installation date.

After the warranty period, the municipality reserves the right to remove any damaged plaques or benches.

Requests for repairs or replacements after the warranty period will be charged to the applicant.

I understand that a donation to the Town of Carman for a bench dedication does not constitute ownership of the item, the land upon which it is situated, or the surrounding area.  Benches will be exposed to natural elements, traffic, and other wear and tear.  The Town of Carman agrees to maintain for as long as possible all dedicated benches against extreme weather damage and vandalism and reserves the right to temporarily or permanently relocate benches in necessary circumstances.
